Internal Fat & Insulin Resistance: The Hidden Cause of Stubborn Weight Gain

If you've been eating less, moving more, and still not seeing the scale budge — or worse, you've lost weight everywhere except your midsection — there's a good chance the problem isn't willpower. It's internal fat.

In more than 12 years of coaching people through weight and lifestyle changes, I've sat across from thousands of people who did everything "right" by conventional advice and still felt stuck. Most of them had never heard the term internal fat. Once they understood it, a lot of their previous frustration finally made sense.

This guide explains what internal fat actually is, why it's so closely tied to insulin resistance, how to recognise the signs in your own body, and — most importantly — what actually helps, beyond another round of calorie counting.

What Is Internal Fat (and How It's Different From the Fat You Can See)

Not all body fat behaves the same way. Broadly, your body stores fat in two places:

Subcutaneous fat sits just under your skin. It's the fat you can pinch on your arms, thighs, or stomach. It's not harmless, but on its own it's not the main driver of metabolic problems.

Internal fat, also called visceral fat, is stored deeper — around and between your abdominal organs, including your liver, pancreas, and intestines. You can't pinch it, and you often can't see it directly, even though it's frequently what gives someone a firm, rounded belly rather than a soft one.

The distinction matters because internal fat isn't just inert storage. It behaves almost like an active organ, releasing inflammatory compounds and hormones directly into the bloodstream that flows to your liver. Harvard Medical School has described visceral fat as a more meaningful health concern than subcutaneous fat precisely because of this metabolic activity, not just its appearance.

This is also why two people can weigh the same and look similar in clothes, yet have very different health risks — one carrying most of their fat subcutaneously, the other carrying a larger share internally.

Where does internal fat actually go?

Internal fat wraps around organs in the abdominal cavity. In excess, it can also infiltrate the liver itself — a separate but related condition often picked up on scans as fatty liver. This is one reason internal fat is sometimes flagged incidentally during an unrelated ultrasound, long before someone notices any symptoms.

How Internal Fat Leads to Insulin Resistance

To understand why internal fat is such a stubborn problem, it helps to understand what insulin actually does.

Insulin is the hormone your pancreas releases to move glucose (sugar) out of your bloodstream and into your cells, where it's used for energy. In a well-functioning system, this happens efficiently — you eat, blood sugar rises, insulin ushers it into cells, blood sugar comes back down.

Internal fat disrupts this process. The compounds it releases interfere with how your cells respond to insulin's signal. Your cells become less sensitive to it — a state known as insulin resistance. Your pancreas compensates by producing more insulin to get the same job done.

Here's the part that catches most people off guard: insulin isn't just a blood-sugar hormone — it's also a fat-storage signal. Chronically elevated insulin makes it easier for your body to store fat, particularly around the abdomen, and harder to release it for energy. That creates a self-reinforcing loop:

  1. Internal fat builds up.
  2. It drives insulin resistance.
  3. Your body produces more insulin to compensate.
  4. Higher insulin makes it easier to store more internal fat.
  5. Repeat.

This loop is a large part of why someone can eat "less" in a general sense and still not lose weight where it matters — if meals are still triggering large insulin spikes, the storage signal stays switched on even in a modest calorie deficit.

It's also why insulin resistance is now understood as an early, often silent, step on the path toward prediabetes and type 2 diabetes. The U.S. National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) describes insulin resistance as a condition where the body doesn't respond to insulin as it should — and notes that it often develops years before blood sugar itself becomes a diagnosable problem.

Internal Fat, BMI, and Why the Scale Can Be Misleading

Body Mass Index (BMI) is useful as a rough population-level screening tool, but it has a well-known blind spot: it can't tell the difference between someone carrying most of their weight as muscle, someone carrying it mostly as subcutaneous fat, and someone carrying a large share of it internally.

Two people can have an identical BMI of, say, 24 — technically "normal" — and have very different metabolic pictures. One might have low internal fat and excellent insulin sensitivity. The other might be the "thin outside, fat inside" profile I mentioned earlier, with internal fat levels that would concern a doctor if actually measured.

This is exactly why so many people who "aren't technically overweight" still deal with fatigue, stubborn midsection fat, or blood sugar patterns that don't add up. The scale and even the BMI chart are measuring total mass — they were never designed to show where that mass is stored, or how metabolically active it is.

In practice, this means a waist measurement (or simply how your clothes fit around the middle) often tells you more about internal fat trends over time than your bathroom scale does. It's not about obsessing over numbers — it's about tracking the right one.

Signs Your Body May Be Carrying Excess Internal Fat

Because internal fat sits deep in the abdomen, you can't always tell just by looking in the mirror. That said, there are patterns worth paying attention to.

Waist size out of proportion to your weight. A waist measurement that's grown even if your overall weight hasn't changed much is one of the more reliable everyday indicators — internal fat tends to show up here before it shows up on the scale.

A firm, rounded stomach rather than a soft one. This isn't a rule you can apply with certainty, but a tighter, more domed abdomen (sometimes people describe it as looking "pregnant" even without significant overall weight) is often a visual clue that fat is sitting deeper than the skin.

Energy crashes 1–2 hours after meals. If you eat and feel a spike of energy followed by a slump, foggy head, or sudden hunger, that pattern can reflect the blood-sugar swings that come with insulin resistance.

Sugar or carbohydrate cravings that feel hard to control, especially in the afternoon or evening — often the downstream effect of those same blood-sugar swings.

Feeling tired despite adequate sleep. Chronically elevated insulin and blood sugar can leave you feeling flat, even when you're technically getting enough hours of rest.

Skin changes, such as darker, slightly velvety patches of skin around the neck or in body folds — a sign called acanthosis nigricans, which is associated with insulin resistance and is worth mentioning to a doctor if you notice it.

Weight that concentrates around the midsection even when the rest of your body doesn't feel particularly overweight — this is the "thin outside, fat inside" (TOFI) pattern I mentioned earlier.

None of these signs are a diagnosis on their own. If several apply to you, it's worth a conversation with your doctor, who can check fasting insulin, blood sugar, or order imaging if appropriate — not to alarm you, but to get an actual picture instead of guessing.

Why Traditional Dieting Often Fails to Reduce Internal Fat

This is the piece that's genuinely changed how I coach people. Most conventional diet advice is built around one number: calories in versus calories out. It's not wrong, exactly — but it's incomplete in a way that matters a lot when internal fat and insulin resistance are part of the picture.

Severe restriction can backfire. Very low-calorie diets often trigger a stress response in the body, which raises cortisol. Cortisol and insulin resistance are closely linked, so a diet that's technically "working" on paper (fewer calories in) can still struggle to shift internal fat if it's simultaneously driving stress hormones up.

Frequent small meals and snacking can keep insulin elevated most of the day. If you're eating every two to three hours, even on "healthy" foods, your insulin may rarely get the chance to come back down between meals — and it's the troughs, not just the food choices, that matter for fat mobilisation.

Not all calories affect insulin equally. A meal built around refined carbohydrates produces a very different insulin response than a similar-calorie meal built around protein, fibre, and healthy fat — even if the calorie count on a label looks comparable.

Exercise alone often isn't matched with the other levers. Someone can train hard four times a week and still carry stubborn internal fat if sleep is poor, stress is high, and meals are still spiking blood sugar the rest of the week. Exercise is genuinely valuable, but on its own it's rarely the whole answer.

This is usually the point in a conversation where people ask me some version of, "So what actually works?" The honest answer is: nothing dramatic, and that's kind of the point.

Common Mistakes I See When People Try to Fix This

A few patterns come up often enough in coaching conversations that they're worth naming directly.

Chasing the scale instead of the waist. Someone does everything right for three weeks, the scale barely moves, and they conclude it's not working — when their waist has actually come down and energy has improved. Internal fat responds to consistency measured in months, not days, and the scale is a noisy, imperfect proxy for what's actually changing underneath.

Adding a punishing workout on top of an already stressful life. Intense exercise is a legitimate stressor on the body. For someone who's already under-slept, overworked, and running on elevated cortisol, bolting on an aggressive training plan can sometimes work against the goal rather than for it. Movement helps — but the type and intensity should fit the person's current stress load, not fight against it.

Treating "diet food" as automatically insulin-friendly. Many packaged "healthy" or "diet" foods are still built around refined carbohydrates and added sugar, just marketed differently. Reading a label for fibre and protein content tells you far more than the word "diet" on the front of the packet.

Giving up after one plateau. Weight and internal fat rarely move in a straight line. A week or two of no visible change, especially around a stressful period, travel, or a cycle change for women, is normal physiology — not evidence that the approach has failed.

Trying to fix everything on day one. The people who stall out fastest are often the ones who try to overhaul sleep, meals, exercise, and stress simultaneously. It's rarely sustainable. Picking the one or two levers that are realistically off track for you right now — and building from there — tends to hold up far better over months.

A Lifestyle-First Approach to Reducing Internal Fat

Rather than another restrictive diet, addressing internal fat comes down to steadying insulin across the whole day, most days — not perfectly, just consistently.

Build meals around protein and fibre first

Protein and fibre slow down how quickly a meal turns into blood sugar, which means a smaller, gentler insulin response. In practice, this can be as simple as making sure vegetables, dal, paneer, eggs, or other protein sources come to the plate before or alongside rice or roti, rather than the meal being carbohydrate-heavy on its own.

Give insulin a chance to come back down between meals

Rather than grazing throughout the day, eating within a defined window with real gaps between meals gives your insulin levels room to settle. This doesn't have to mean formal intermittent fasting — even moving from six small meals a day to three balanced ones can make a noticeable difference for some people.

Move daily, not just at the gym

Regular movement — walking after meals in particular — helps muscles take up glucose without needing as much insulin to do it. A 10–15 minute walk after your largest meal is one of the highest-value, lowest-effort habits I coach people on, precisely because it targets the exact mechanism at play here.

Protect your sleep

Even a few nights of short or poor-quality sleep measurably reduces insulin sensitivity. If weight isn't shifting despite "doing everything right" during the day, sleep is one of the first things worth auditing honestly.

Manage stress as a metabolic habit, not an afterthought

Chronic stress keeps cortisol elevated, and cortisol works against insulin sensitivity. This doesn't mean stress has to disappear — that's not realistic for most people's lives — but building in daily decompression (even 10 minutes of a walk without your phone, or slow breathing before bed) is a legitimate part of an internal-fat strategy, not a "nice to have" on the side.

None of these changes need to happen perfectly or all at once. In the community I work with, the people who make lasting progress are almost never the ones who overhaul everything in one week — they're the ones who pick two or three of these and stay consistent long enough for the body to respond.

What this can look like in a real, busy day

It helps to see this as an ordinary day rather than a plan that requires a complete lifestyle reset:

  • Morning: A protein-forward breakfast (eggs, paneer, sprouts, or a dal-based option) instead of only tea and something sugary or refined. This alone changes how the rest of the morning's blood sugar behaves.
  • Mid-morning: No grazing between breakfast and lunch. If hunger genuinely shows up, a small handful of nuts or a piece of fruit is a reasonable bridge — the goal is fewer, steadier meals, not rigid rules.
  • Lunch: Vegetables and protein first on the plate, rice or roti alongside rather than the entire portion. A 10-minute walk afterward if the day allows it, even just around the office or the house.
  • Evening: A short walk after dinner rather than sitting immediately, and a wind-down routine that doesn't involve a screen right up to the moment of sleep — this protects the sleep quality that insulin sensitivity depends on.
  • Across the week: Two or three sessions of some form of strength or resistance movement, at an intensity that feels sustainable rather than depleting.

Nothing here requires a gym membership, a specific diet plan, or giving up rice and roti entirely. That's deliberate — the approach has to survive contact with an actual week, including the days that don't go as planned, or it isn't really an approach at all.

Why consistency matters more than intensity here

Because internal fat and insulin resistance built up gradually — often over years — they also resolve gradually. A single "perfect" week rarely moves the needle much, and a single "bad" week (a wedding, a work deadline, a holiday) rarely undoes real progress either. What matters is the average pattern across a month, not any individual day. This is one of the hardest mindset shifts for people who are used to thinking about weight loss in terms of short, intense pushes — and it's usually the shift that makes the difference between temporary results and change that actually holds.

A simple fasting blood glucose and insulin test, or in some cases imaging, can give you real data instead of guesswork. If you're already managing diabetes, thyroid disease, or another condition, loop your doctor in before making significant changes to eating patterns, fasting windows, or exercise intensity.
